Community Care Navigation

Pilgrims Hospice Society is pleased to offer Community Care Navigation — a first-of-its-kind program in Edmonton and the surrounding areas.

Community Care Navigation is a free service for individuals with advanced illnesses and their families. Working with a variety of care organizations, we help clients access the care they need and deserve. From diagnosis through treatment and into end-of-life care, our guidance and compassionate support help you understand your care options, connect with resources and make informed decisions with confidence.

YOUR CARE, YOUR WAY

Navigating the complexities of health care can feel overwhelming, especially for palliative patients whose needs often differ from other patients. Whether you’re making decisions about medical care, planning for the future or looking for emotional support, we can help. We’re here to answer your questions, connect you with the right resources, and make sure you feel informed and supported along the way.

We can help with:

  • Answering questions about your care and treatment options (e.g., the Cross Cancer Institute, the Pain & Symptom Team, Palliative Home Care, hospice) and when to transition between them.
  • Explaining medical terms and what to expect
  • Pet care and rehoming
  • Grief support and spiritual care
  • Advanced Care Planning information
  • Finding a primary care doctor who specializes in palliative care
  • Referrals to community services

WHO WE SUPPORT

Community Care Navigation is available to adults (age 18+) who:

  • Have a palliative diagnosis (e.g., cancer, organ failure)
  • Live in the Edmonton area (including Sherwood Park, St. Albert, Beaumont, Leduc, Spruce Grove, Stony Plain and Devon)
