HOME VISITING HUB
Pilgrims Hospice Society is pleased to offer a Home Visiting Hub Program, which provides home visits from primary palliative care physicians. This service bridges the gap for those living with a life-threatening progressive illness who still live at home but do not have access to a primary care provider.
WE COME TO YOU
Working closely with palliative home care and palliative care specialists, our team of physicians and nurses supports clients during their journey with a life-threatening and progressive illness.
When you receive care from the Home Visiting Hub, one of our physicians becomes your primary care provider. The care you receive includes:
- Regular home visits and daytime nursing support by phone (weekdays)
- Access to an on-call physician (evenings, weekends and holidays)
- Symptom monitoring and management (e.g., pain, nausea, shortness of breath)
- Prescriptions and refills
- Guidance and support with disease progression and other health concerns
- Connection to community services (e.g., home care, Cross Cancer Institute)
- Coordination with other health care professionals
- Goals of care discussions
- Support for death in the home or transition to hospice
- Guidance and support for caregivers
WHO WE CARE FOR
The Home Visiting Hub provides care to individuals in Edmonton and the surrounding areas, including St. Albert, Sherwood Park, Spruce Grove, Devon, Beaumont and Leduc. Patients outside of these areas will be considered on a case-by-case basis.
To be eligible to receive care from our Home Visiting Hub, an individual must:
- Be at least 18 years of age
- Have a palliative, life-limiting or terminal diagnosis
- Need a primary care physician or one who does home visits
Please Note: The patient and their family (if applicable) must agree to have a Home Visiting Hub physician take over their care and to contact their current physician to inform them of the change.

EXISTING PATIENTS
We will return your message within one business day (sooner if the matter is urgent).
Please have the following information ready:
- Patient name
- Name of Home Visiting Hub physician
- Your name, number and relationship to the patient
- Reason for your call (please be as specific as possible)
Examples of non-urgent calls: Rescheduling an appointment, non-urgent prescription renewal, message for a physician, minor change in health status, expected death in the home.
Examples of urgent calls: Pain crisis that has not improved with breakthrough medications, sudden change in consciousness, uncontrolled symptoms, severe discomfort.
